Tirzepatide: A guide to the dual GLP-1 + GIP receptor agonist

Medically reviewed by: Op. Dr. Ferhat İlen, ENT – Head & Neck Surgeon · Last updated:

Tirzepatide is a once-weekly administered medication that acts as a dual GIP and GLP-1 receptor agonist. In the SURMOUNT-1 clinical trial, an average weight loss of 20.9% was reported at 72 weeks with the highest dose.

Regulatory / evidence status: FDA-approved: type 2 diabetes (Mounjaro, 2022), weight management (Zepbound, 2023), and obstructive sleep apnea (2024).

How does it work?

It slows gastric emptying, enhances satiety, and regulates blood glucose levels via glucose-dependent insulin secretion. The GIP component additionally influences adipose tissue metabolism.

Why does muscle loss matter?

During rapid weight loss, a proportion of the lost weight can come from lean muscle mass. Therefore, adequate protein intake, resistance training, and body composition monitoring are integral parts of therapy.

Who is it not suitable for?

A personal or family history of medullary thyroid carcinoma or MEN2, pregnancy, a history of severe pancreatitis, and certain gastrointestinal disorders require careful medical evaluation.

Side effects

Nausea, diarrhea, constipation, and decreased appetite are the most common adverse effects, typically most pronounced during dose escalation. Gradual dose titration improves clinical tolerance.

Frequently asked questions

Do you regain weight after stopping tirzepatide?

Clinical trials have shown that a substantial portion of lost weight can be regained once treatment is discontinued; therefore, long-term lifestyle and maintenance strategies are essential.

Is it more effective than semaglutide?

In the SURMOUNT-5 head-to-head trial, tirzepatide demonstrated greater average weight reduction; individual selection should be made collaboratively with a physician.
